Boost Diablo Immortal offers ways of permanently boosting characters using Legendary Gems, which are obtainable in game (with very low droprates for free-to-play Players, even for the worst 1* kind) as well as via real money transactions using Eternal Orbs or in Rifts using "Legendary Crests" which guarantee a Legendary Gem drop for each Legendary Crest used (up to 10 per Rift).
Cosmetic There are many cosmetics purchasable with Eternal Orbs, which are the game's real money equivalent currency.
Currency Diablo Immortal has two premium currencies; Platinum and Eternal Orbs. Platinum is both obtainable in game (but at a very slow and not viable pace for bigger purchases) and purchasable directly for real money. Eternal Orbs are only purchasable for real money.
Free-to-grind Most gear, but not all (see "Unlock" for example), can be obtained by free players, the odds are just very much not in their favour.
Infinite spend There is no clear ceiling in Diablo Immortal regarding how much one can spend on premium currency and microtransactions.
Loot box Diablo Immortal has lootboxes, and an opportunity to buy a lootbox with real money is one of 'rewards' for certain content.
It also has Elder Rifts. which are dungeons with random loot, where you can spend money to boost odds or guarantee rare drops using 'crests', which are in turn purchasable via Elder Orbs, the premium currency purchasable with real money transactions. There is notably a second type of Legendary Crest, one with the "Eternal" modifier, that is only purchasable with real currency, which is the only way to acquire Legendary Gem drops that are not bound to the character they dropped for.
Player trading The in-game market uses only Platinum (not gold, the normal, in-game-only currency), and since Platinum is purchasable via real money transactions, players can effectively buy items from other players for real money.
Time-limited 'Boon of Plenty' service offers time limited premium items. Some of the premium items you bought with real money will even disappear unless you log in daily for a certain period of time (usually 30 days).
Unlock Diablo Immortal usually offers "limited" Packs to buy at the Shop after finishing a dungeon. They start at US$0.99, pressuring you to buy into them with Displays like "+800% Value" and "limited offer".
After completing more dungeons, the price of those offers will increase significantly (up to US$29.99) while the "+xxx% value" decreases more and more.
Those offers often contain packs of Eternal Orbs that are so few in number, that you are unable to purchase anything with it, unless you are spending more money to buy something from the Shop.
For example, as the only way to "Awaken" a Rank 10 - legendary Gem which significantly boosts its value and Stats for your Character, you need an "echo of awakening", which can only be obtained for 1,000 Eternal Orbs. You can either buy 2x 630 Orbs for US$19.98 (US$9.99 each) or 1.650 Orbs for US$24.99. This way Diablo Immortal makes sure, you have always some Orbs left, to trigger you into buying some more.
According to one calculation, it costs more than US$540,000 to fully gear up one character with best-in-slot items in Diablo Immortal.[4][5]
