Do everything just like in Medal of Honor: Allied Assault#Max quality graphics settings but you must use mainta path instead of main
And remember not to run configure_spearhead.exe in future, as it will reset those settings.
Save game location
<install dir>\MOHAA\mainta\save\
Config files are located in <install dir>\mainta\configs\unnamedsoldier.cfg and <install dir>\mainta\newconfig.cfg
Issues And Fixes
Graphics settings are lost each time when starting game
You need to copy your graphics settings from <install dir>\mainta\configs\unnamedsoldier.cfg to <install dir>\mainta\newconfig.cfg
Or you can use Max quality graphics settings
