  • User: [[User:Username]] [[User_talk:Username]]
  • Offense: Cite the specific Code of conduct rules broken
  • Diffs: Link to specific diffs
  • (Moderators only) Result: Pending or list delivered punishments/outcomes, any ban 'reason' should link to the Moderation noticeboard Topic.
